The death of any child is a tragedy. The Department of Children and Families and our community partners are saddened by the senseless death of Ronderique Anderson. This quality assurance review was completed to provide a comprehensive assessment of this case and to provide us with the insight necessary to do a better job at ensuring the safety and well-being of the children in our care. In order to improve the safety of our children and the quality of our work, I am directing my staff and Hillsborough Kids, Inc., along with their case management organizations, to do the following:

  • Assure the level of family engagement is adequate to gather sufficient information needed to make informed decisions regarding child safety and case planning
  • Improve the quality of supervisory oversight and guidance for all cases to enable better decision-making, including a critical review before closure
  • Ensure that the family preservation protocols are followed by making sure parents and children are appropriately engaged in the services they need
  • Complete a review of all home studies conducted for families with young children and institute ongoing controls to make certain that all home studies reach conclusions that are safe for children
  • Review all cases on children up to age five to ensure that case plans and services are meeting their needs
  • Involve the Attorney General’s office in permanency staffings
  • Implement better structured decision-making procedures to flag high-risk cases for special oversight by the lead agency

I expect to receive frequent updates regarding the progress of implementing these improvements, and I expect the corrective actions to be completed by April 30. I believe the lessons learned from this case will move our system forward in a way that ensures our children remain safe, healthy, and nurtured. We owe it to Ronderique and his family to do no less.
