To download Provider Information, make the appropriate selections for Program Type and County. At least one checkbox must be selected in each category.

Selecting All Program Types or All Counties generates a large file and may take sometime to download, so please make your selections carefully.

Once selections Program Type and County are made, selecting the Submit button begins the download process. Selecting the Reset button removes all Program Type and County selections.

Important Notice!

The Provider Information data results are displayed in an Excel 2003 spreadsheet. Users running the 2007 Microsoft Office system must use the Internet Explorer browser Compatibility View to access the provider data. The process is easy:

Compatibility View
  1. Go to the browser address bar and click the 'Compatibility View' button displayed to the right of the address bar.
  2. Select Program Type and County. *
  3. Click Submit.
  4. Open or Save xls results.
* NOTE: At least one Program Type and one County selection must be selected to proceed.
           

Information contained in the Excel spreadsheet consists of County, Program Type, Facility or Home Name, Address, City, State, Zip (5 digits only) and Capacity. The sequence of each is by County, Program Type and Facility or Home Name. Please note if no records were found that match the selection criteria, the spreadsheet would contain one row as described in the example below.
